I know a little about this.

Synthetic EPO is the man-made version of the naturally occurring hormone erythropoietin.

The medical purpose for Synthetic EPO is to treat anemia a medical condition where the body does not naturally produce enough organic, natural EPO.

Natural EPO is a hormone produced by the kidneys. It stimulates the production of red blood cells.

Red Blood cells carry oxygen through the body.

More red blood cells = more oxygen to muscles = enhanced stamina

Synthetic EPO given to race horses serves no legitimate medical purpose. It is performance enhancing because it artificially increases stamina.

Side effects include, most notably,  blood clots and other cardiovascular issues.

As I understand it, EPO goes to work quickly, and has an extremely short half-life.  The effects last quite a while.
It takes out-of-competition testing and a great deal of good luck to catch anybody.  And it ain't cheap to test for it.
It pains me greatly to say it, but I have to believe that a HUGE number of "high performance trainers" have it down to a science and use EPO routinely.  And that's sad.

You know what else is expensive? Owners not showing up to buy yearlings because they have had enough. Or Owners not participating in the claiming game all winter which keeps the game alive. This industry has zero intelligent people and will go the way of the dodo bird in short order. Canada is basically a fair circuit with the exception of Mohawk and that place will be down to two or three days of racing in the next year. And while things are apparently well down here if you take a deeper dive, the numbers indicate something different. Amount of registered owners has decreased 20% over the past five years and if that isn't alarming in itself, handle adjusted for inflation has decreased 40% in the past decade alone. I suspect Kentucky will boom just like Ontario did when slots were introduced but if we follow a similar approach where there is zero improvement to the betting and live product it too will be short lived. Testing for EPO/Cobalt/Aranesp/TCO2 should be mandatory for a year---call it a trial ballon---and all trainer's numbers should be evaluated. From there you send a very strong message and get things at least to the point where an owner doesn't have to pay 6-7K a month to compete as that isn't sustainable.

Just go to Middletown they pass it around like it's candy.  Stratton, Adam, Bako, owners pull horses when they don't function and Bako buys them back and switches ownership.  Trotter who raced recently comes to mind and let's not forget the horse who was with Bako, went to another trainer couldn't go and now is owned by Bako.  Ford behind the whole thing, not hard to figure.
