Abbreviation Codes DX ?

In the race charts it shows  "DX" . Looked it up in USTA and it says "Driver caused break". How would they know so soon after the race and posting the charts and how does a driver cause a break? What would be a few examples a driver caused break?  (Harrahs Philadelphia Race 4 09/20/19.) And does the plain "X" mean the horse caused the break?
